Professional Care Management Services

Case management is essential to providing comprehensive care to patients struggling with addiction. At Northwest Community Health Center, we understand that opioid addiction can be a challenging and complex issue requiring a multi-faceted treatment approach. Our case management team is trained to provide individualized support to patients seeking help with opioid addiction. One of the tools we use to help break the cycle of addiction is medication-assisted therapy (MAT). MAT combines medication with behavioral therapy and support to help patients overcome addiction and maintain long-term recovery. Our team works closely with patients to develop personalized treatment plans that may include buprenorphine, methadone, or naltrexone. These medications can help reduce cravings and ease withdrawal symptoms, allowing patients to focus on their recovery. Our case managers also provide ongoing support and monitoring to ensure that patients are following their treatment plan and making progress in their recovery. They may work with patients to develop strategies for managing cravings and stress and provide referrals to other services such as counseling, support groups, and job training programs. Navigating today’s healthcare system is complicated and at times can be overwhelming. As your patient centered medical home, we understand the importance of having a central place to access information when coordinating your care and have individuals who work within your team to be this contact point if and when you need them. 

 

Case Managers are able to follow up with patients at regular intervals, provide self-management support, connection to social services, and care coordination including assistance with appointment scheduling and finding solutions to transportation needs.

 

Case Managers focus on the Entire patient (social, financial and health) rather than a specific disease. They work with patients to make an individualized care plan and goal setting.
